I understand that Pilates and related exercise activities involve physical exertion and may carry a risk of injury. I confirm that I am voluntarily participating in Pilates classes and activities offered by Starr Pilates*

I understand that participation may involve risks including, but not limited to, muscle soreness, strains, sprains, falls, aggravation of existing conditions, and other injuries.*

To the fullest extent permitted by applicable law, I release and agree not to hold Starr Pilates, its owners, employees, instructors, contractors, and agents responsible for claims arising from my participation in studio activities, except to the extent such liability cannot legally be waived or released.*

Safe Participation

I agree to:

• Follow the instructor's directions and studio safety rules.

• Use Pilates equipment only as instructed.

• Inform the instructor if I experience pain, dizziness, shortness of breath, or other unusual symptoms.

• Stop exercising if I feel unable to continue safely.

• Inform the instructor of any relevant change in my health or physical condition.
